Elm Wood Center provides both LongTerm Care and ShortStay Rehabilitation. We offer Skilled Nursing, Palliative Care, Hospice Care, Respite Stays, Medical and Rehabilitative Care for patients and residents. Elm Wood Center provides daily therapy services including Speech Therapy, Occupational Therapy and Physical Therapy along with 24-hour nursing care. Elm Wood has a spacious dining room offering restaurant-style dining and patients and residents are offered a choice for meals, including an always available menu.

Kaylyn Rushford, LNHA, MHA
AdministratorKaylyn is excited to join Elm Wood Center. Her leadership philosophy centers on a deep passion for the person-centered care model. She firmly believes that quality care starts with understanding and addressing the unique needs and preferences of each individual resident. This approach extends to their focus on team development; Kaylyn is passionate about fostering a supportive environment where staff can grow professionally, ensuring the entire team is empowered to deliver the highest standard of care possible. Kaylyn is driven by the goal of making Elm Wood Center a leading example of compassionate and high-quality nursing care. Outside of her professional life, Kaylyn values family time and can often be found going on adventures with her kids and dogs, embracing the simple joys of outdoor exploration and quality time together.

Kasra Djalayer
Medical DirectorKasra Djalayer, MD earned his medical degree from the Complutense University of Madrid in 1989. He completed a residency with Yale-Griffin Prevention Research Center in Connecticut, a fellowship in rheumatology and clinical immunology at the University of Vermont, a mini-fellowship in geriatric medicine from Stanford University, and a postgraduate sub-specialty diploma in rheumatology and clinical immunology from the University of South Wales, London, followed by a fellowship in disability medicine and functioned as a Senior Diplomate in this field of medicine. He also studied obesity medicine at Harvard University. Additionally, Dr. Djalayer received an Executive Master of Business Administration degree, an International Masters for Health Leadership degree, a mini Master of Business Administration degree, and a diploma in advanced and executive management from McGill University. Following his training, the Dr. Djalayer attained board certification in internal medicine through the American Board of Internal Medicine (ABIM). He is proud to be the recipient of the Most Compassionate Doctor Award and Patient Choice Doctor Award each year from 2011 to 2015. His experience includes outpatient primary care, acute care hospitalist care, and Medical Director and attending physician in skilled nursing facilities.

Elizabeth (Beth) Dow
Family Nurse PractitionerBeth is a Board-Certified Advanced Practice Registered Nurse with a family practice concentration. She went to Saint Elizabeth’s School of Nursing, received her BSN at Emmanuel College, and Master’s Degree at Southern NH University for general nursing focus. Beth then completed her Master’s at United States University of San Diego for her APRN- Family Nurse Practitioner. She designs tailored plans of care with each resident to meet and exceed individualized goals. Beth is available in house many days a week seeing her residents. She is an amazing addition to the medical staff at Elm Wood Center. We all love and appreciate her greatly!

Maryellen Corliss, RN BSN
Director of NursingMaryellen is a local Claremont resident with 24 years of nursing experience, including direct care, leadership and management. She worked in hospice for 18 years before finding her new home at Elm Wood Center in longterm care. She has a passion for excellence and quality in her nursing practice and leadership, and believes strongly in leading by example. "Team Work is The Dream Work" is a motto she lives by. She holds a Bachelor's degree in Nursing from American Sentinel University. She has been married for 25 years, and has 2 grown daughters. In her down time, she is a die hard Patriots fan, enjoys rides on back roads, and trips to the ocean.

Bryanna Cook
Admissions & Marketing DirectorBryanna has been with the Company for 10 years. She has been working in the medical field for 15 years. She worked in our local hospital during this time as well. She attended River Valley Community College. She has worked in all medical settings from being an LNA, a social worker, even front desk check-in. She loves to work hard and help in any way possible. She feels it is the most beneficial and rewarding feeling helping people. We are so happy to have her on our team!

Daniel Young, OTA/L
Director of RehabilitationDaniel has been in the human services field since 1999, working at DSSC, WKS Inc in Keene as well as Crotched Mountain Rehab. During that period, he received his BS in Business Management from Franklin Pierce College as well as his Associate Degree in Occupational Therapy from River Valley Community College. Daniel has been a therapist since 2010. He is a veteran of Desert Storm serving in the US Navy, and is married with 2 children.
Total Knee Replacement Success Story
Maria had both her knees replaced on December 14th. After her stay in the hospital, Maria came to Elm Wood Center for continued therapy because she needed help with just about everything including her Activities of Daily Living. Maria was challenged by her level of pain, making it difficult for her to stand or ambulate. After 8 weeks of intense therapy, Maria had progressed by leaps and bounds. She walked the halls on her own before she went into the rehab room for therapy. She was able to do her ADL’s independently, and she was seen making cookies in the rehab kitchen. Maria was able to successfully return home on February 1st and after checking in with her, she has been doing well at home since leaving Elm Wood Center.
