It is hard to believe that the holiday season is fast approaching. We know that spending the holiday season with friends and family is always important, especially this time of year. That’s why we are working hard to make it as special as possible despite the ongoing pandemic.
As we plan for this year’s fall and winter holiday celebrations, safety remains our primary focus as we work to protect your loved ones, our staff, and families. This year we will all be celebrating differently as celebrations will not be in large gatherings. Unfortunately we will not be able to offer opportunities for families to share meals within the Center with loved ones or large resident and family gatherings. As recommended by the Centers for Medicare and Medicaid Services (CMS), a federal regulatory agency, we also strongly discourage resident Leaves of Absence, to protect both the resident who might be exposed to coronavirus when outside the Center, as well as all other residents when somebody returns from a leave of absence. read more
Hammonds Lane Center
613 Hammonds Lane, Brooklyn Park, MD 21225
Hammonds Lane Center is conveniently located in Northern Anne Arundel County in the heart of Brooklyn Park. We are minutes outside of Baltimore, and accessible from the 695 Beltway, Route 2 (Ritchie Hwy), and minutes from I-97. We are a Skilled Nursing Facility offering ShortStay Rehabilitation Services along with LongTerm Care Services. Other services include Respite Care, Hospice Care, and VitalStim which provides additional modalities of treatment. Hammonds Lane Center is well known for their spacious dining room that offers restaurant-style dining. Cafe Hammonds offers many meal options, and prepared by our in-house Chef. Our gourmet meals are available to both our staff and families along with homemade desserts. In addition, Hammonds "Country Cottage", a Gift Shop providing various snacks, drinks, and gifts, is overseen by our own Resident Council. Hammonds Lane also offers Salon Services.

Leanna LennoxCenter Executive Director
Leanna has worked in the long-term care industry for over twenty-five years. She has proudly served fourteen of those years as a Nursing Home Administrator. Leanna graduated from the University of Phoenix in 2004 with a degree in Business Administration. She has a strong passion for the care her residents receive. She feels the facility and residents must work together to create a caring and compassionate environment. Leanna enjoys spending time with her family and going to the beach.
Dr. Tana VivianoMedical Director
Dr. Viviano received her Medical Degree at the University of Connecticut Health Center. She completed Internship at Brown University in Providence, Rhode Island and completed the remainder of Residency at the University of Connecticut. She is Board Certified in Internal Medicine and is a Certified Medical Director for skilled nursing and long-term care facilities. She has practiced in the long-term care and skilled nursing arena for almost 10 years. She is employed by Adfinitas Health and has been an attending and Medical Director in Genesis facilities for over 5 years. She enjoys the variety of patients that she meets in the subacute rehabilitation setting and also enjoys the relationships that develop with the long-term residents as their primary care doctor.
Terri GayhardtDirector of Nursing
Terri graduated with a degree in nursing in 1987, completing a BS in Behavioral Science. She has worked at University of Maryland, Mercy and John’s Hopkins hospitals, specializing in Labor and Delivery for 18 years, then began working in senior care in 2004 as Director of Clinical services for a highly reputable homecare agency. Terri moved to Director of Nursing for the largest Assisted Living facility in Howard County for three years, then started in LTC and sub-acute rehab in 2012 as Director of Nursing for a large facility in Towson. She transitioned to Genesis in June 2016 at Caton Manor and moved to Hammond’s Lane Center in January 2017. Holds advanced certifications in Childbirth education, Case Manager/Delegating Nurse and Infection Preventionist. Terri resides in Ellicott City, MD where she raised five children. Her hobbies are reading, golf, dancing and music.
Debbie LotzAdmissions Director
Debbie has been with Hammonds Lane Center for the past 11 years and has been in the health care field for over 20 years. She was the Environmental Services Director for four years and during that time has been recognized in achieving the "Gold Key Award" for each of the four years she held that position. In July of 2004, Debbie then transferred to the Admissions Director position where she has been for the past seven years. She was also a recipient of the Census Development Quality Key Award. She is customer service oriented and dedicated to both the residents and family members' needs . As the point of contact for many of our family members and residents, Debbie has given them an easy transition to Hammonds Lane Center.
Karen MartzDirector of Rehabilitation
Karen is an Occupational Therapist and has worked for Genesis Rehab Services for 20 years. Karen earned her degree from Towson University and is currently the Director of Rehabilitation at Hammonds Lane Center. She enjoys working with patients and helping them improve their functional abilities and safety.

"My stay at Hammonds Lane Center was very nice. I met a lot of great people and the care was more than adequate. Everyone I met was very helpful and friendly. My rehab met a lot of my needs. The exercises I do are very beneficial and I plan on doing them at home. Thank you everybody".
Caring for myself again
I was a patient here at Genesis Hammonds Lane Center. Once I arrived to the facility I was unable to stand and walk. Now I can walk with a walker up to 300 feet! I can bathe, and dress myself and go up and down steps.

"I came in on a total hip replacement, brand new hip, and wasn't able to walk. Patient staff here was very professional in dealing with me. Monique, Krysten, Rachael, Marc never gave up. I rate the PT up to "Kernan". I know I was there over a year. I give the PT staff here the best of the best, I recommend the best to all of them."

Rehab never gave up on me
I came to Hammonds Lane with a total hip replacement, brand new hip and was not able to walk. The staff here was very professional , even though I was not a pleasant person at times. Rehab never gave up on me! I rate them up to "KERNAN" Patient now is walking with a SPC and able to negotiate steps.

I progressed well
When I came to Hammonds Lane Center with an extensive Cardiac history. I progressed from bed bound, non ambulatory status to independent bathing and dressing and amubulating with a rolling walker.

When I first came here to Hammonds Lane I did not think I was going to get through this. Because of the care of the Therapy department staff they were awesome and they really do care about you.

I really looked forward to Rehab and the visitation with the rehab staff especially since I can't see my family. Overall, I am doing well and I am now able to work 80ft using a rolling walker.

Very good physical therapy
My stay here at Hammonds Lane was pretty good. The only problem was a little delay in response time to the call bell. All in all this is a good place and the physical therapy staff were very good to me. My aids were wonderful to me. I really have few complaints, this is generally a good place ! I was kept clean and well attended to.

OT & PT Therapy was great in getting me ready to go home! I did everything they told me to do in preparation for home. Also the food to give me strength was also good! I was able to walk when I was ready for discharge good job!
