Tygart Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care. We accept Medicare, Medicaid, and most private insurances. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. Tygart Center is a We Honor Veterans community partner and is VA contracted!

Chris Marshall
AdministratorChris is a West Virginia native and a graduate of West Virginia University where he earned his Bachelor’s Degree in Business Administration. His journey with Genesis began in 2011 as an Administrator in Training at Hilltop Center and Teays Valley Center, both located in Southern West Virginia. Since then, Chris has served as the Administrator in nine different buildings across West Virginia for Genesis HealthCare. In 2014, Chris was 1 of 30 Administrators chosen nationwide to attend the National Emerging Leadership Summit for Long Term Care Administrators in Washington, DC. In 2019, Tygart Center was honored to have Chris assume his leadership role, as he is highly respected and appreciated for his excellent leadership skills. Chris and his fiancée, Blaire reside in Morgantown where they enjoy cheering on the Mountaineers.
Dr. Nick Mains, DO
Medical DirectorNick grew up in southwestern Pennsylvania, and he is a graduate with a Biology degree from Allegheny College in Meadville, PA. He then moved on to Edward Via Virginia College of Osteopathic Medicine where he earned his medical degree in 2014. He then migrated back north to Morgantown, WV, where he still resides, to complete a Medicine-Pediatrics residency at WVU Medicine. He is double board certified in Internal Medicine and Pediatrics. He aspires to provide the highest quality of care with compassion to his patients. He enjoys spending time with his family outside of work.
Brianna “ Bree” Craynon
Director of Marketing and AdmissionsBree is the Director of Marketing and Admissions at Tygart Center and has been a part of the Genesis family since 2020. She attended WVU and has 12 years of medical experience working in surgical, hospital, and outpatient settings. Her favorite part of her job is helping families find the care that their loved ones deserve. Bree is from West Virginia and lives in Jakes Run with her husband and two dogs. She enjoys gardening, traveling with her friends, and going to concerts.
Trinity Celapino
Director of RehabilitationTrinity is a licensed physical therapist assistant earning her Associates in Applied Science Degree from Pierpont Community and Technical College in 2013. In 2013, she joined the Genesis Rehab Services family as a PTA at the Pierpont Center. After working there for 5 years, she transitioned to DOR to fulfill the position at the Tygart Center. Trinity is originally from Pennsylvania, but in 2005 made Fairmont her home along with her husband and 2 daughters. When not working, Trinity enjoys spending time with her family and enjoying nature.
Kelvin Nix
Dietary DirectorKelvin Nix is our dietary director. He resides in Shinnston and brings 18 years of cooking and restaurant management experience to Tygart. He loves his job here at Tygart Center and says the residents are awesome and the best part of his day is interacting with them and attending to all of their dietary needs. Kelvin enjoys spending his free time watching movies and being with friends.
