Magued Rizkalla, MD, FACP, MPH, CMD
Medical DirectorDr. Magued Rizkalla has served as the Medical Director at Rochester Manor since October 2017. He brings multiple years of experience in the long term care field.

He received his medical degree from Cairo University, graduating in the top 1% of his class. He received a Master of Public Health degree from Boston University, School of Public Health. He completed his residency in internal medicine at St. Elizabeth’s Medical Center of Boston, his residency in neurology at the University of Vermont and his residency in transitional medicine at Metro West Medical Center in Framingham, MA.
When not taking care of his patients, he enjoys spending time with his wife and 3 children.

Anne Munck, RN
Director of NursingAnne comes to Rochester Manor with over 20 years in nursing, including 8 plus years as a Director of Nursing. She became passionate about working in the geriatric field while caring for her elderly grandmother with Alzheimer’s at age 18 as a nursing assistant. Anne believes that the quality of life of the residents should be foremost and that dignity & respect are hallmarks of caring for the aging. Her clinical experience extends to short-term skilled nursing as well, and she strives to impart a customer service mindset “from the top down.”

Daphne Chiavaras
Director of AdmissionsAs the Admissions Director at Rochester Manor, Daphne Chiavaras works closely with individuals and families to help them navigate short-term rehabilitation or long-term care with understanding and support. She has been in healthcare since 1990 and has served as an Admissions Director since 2007, building strong relationships with patients, families, hospitals, and community partners along the way.
Daphne values the opportunity to guide people through each step with compassion and clarity, helping to make every experience at Rochester Manor as comfortable and reassuring as possible. Whether someone chooses Rochester Manor or another level of care that best fits their needs, she believes it is an important part of her role to help each family find the right path of support.

Jeremy Cabanel
Director of RehabilitationJeremy is new to the Company having moved from Seattle in 2018 to act as the Director of Rehabilitation at Rochester Manor. Jeremy is an Occupational Therapist and graduated from Sacred Heart University in 2012. Previously, Jeremy has worked in pediatrics, skilled nursing, home health and acute care hospitals in New Orleans and Seattle. His passion for the elderly drew him back to skilled nursing where he manages an excellent team of Occupational, Physical, and Speech therapists.

Brie Szidat
Senior Director of Social ServicesBrie Szidat is the Senior Director of Social Services at Rochester Manor, where she has been part of the team for nearly five years. With over 13 years of experience in social work and leadership roles within rehabilitation and long-term care settings, Brie is dedicated to supporting residents through every stage of their journey—whether they are preparing to return home or receiving long-term care. She values witnessing each resident’s growth and success in their own way and takes pride in helping them and their families feel at home. Brie appreciates being part of a compassionate, collaborative team that truly strives to make a difference in people’s lives. Outside of work, she enjoys spending time with family and friends, attending concerts and sporting events, and once earned a full scholarship to play basketball at the University of Lowell.

Eryn Plata
Physician AssistantOriginally from Maine, Eryn earned her Bachelor of Science in General Biology with a minor in Spanish from the University of New Hampshire, which included a semester abroad in Costa Rica. She went on to complete her Master’s Degree in Physician Assistant Studies at Samuel Merritt University in Oakland, California. Eryn brings 14 years of experience as a Physician Assistant, including one year in Family Medicine, nine years in Emergency Medicine, and three years in Cardiology. She joined the Rochester Manor team in 2024. Outside of work, Eryn enjoys spending time outdoors and traveling with her husband and children.

Kindness and Compassion
My husband had a recent respite stay at Rochester Manor. He had a great roommate, that set my mind at ease while I was on vacation and boy, did I need it! My children said everyone was great and he seemed happy during his stay. He was even walking better when I took him home and gives us something to continue working on. Great Stay!
Today we said farewell to Ronald F. after having been at Rochester Manor since April 2020. He had set a goal for himself that he was getting home and although it took a while, he did it! He stated: "I had a great stay, despite the circumstances of not being able to move out of bed for the first 5 months I was here. With the help of PT (Deanna and Pat) they got me to the position of where I'm at now...walking! Thank you to all of the staff at Rochester Manor. I appreciate all of you!"

The Rochester Manor has excellent staff, Nurses, LNAs, all employees from the top down are caring and efficient. They all pitch in wherever they are needed to make the residents comfortable. All have shown their excellence particularly during this past year of COVID-19 going above and beyond to make sure the virus remained outside their doors, and with complicated planning strategies to be sure families had visits with their loved ones whenever it could be done safely. Positive Outcome
Richard admitted to Rochester Manor on 8/23/21 from a local ER where he presented following a fall at home. He has a history of diabetes, stroke, coronary artery disease. Although he did not meet the criteria to admit to the hospital, he was able to admit for skilled nursing and rehab services under the Medicare Waiver Program! He lives alone and both hospital staff and center staff felt he would benefit from these services and Richard was in agreement. During his stay he received nursing care, physical and occupational therapy. Upon admission he required supervision with transfers with his rollator and at time of discharge he was independent with this. Good luck Richard, we wish you the best!
