Sandy River Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from an illness or injury. We accept Medicare, Medicaid, and most private insurances. Joanne Shaw, M.S.M., LNHA
AdministratorJoanne Shaw began serving as the Administrator for Sandy River Nursing Center in October of 2021. Her clinical and administrative career spans more than 35 years and includes transforming acute, sub-acute, and long term care teams with a focus on innovating leadership and process that requires improved performance, energy and drive. She has held Regional leadership positions and served on numerous healthcare boards and committees and has been a licensed nursing home administrator since 2000. She holds a Master’s Degree in Health Care Administration Management from Southern New Hampshire University and a BS in Health Care Administration from Southern New University. Past accomplishments include Executive Director of the Year, Area Executive, AHCA Facility Quality Award recipient, Above & Beyond District Facility Leader Award. She has been a member of the Massachusetts Chapter since 2007 and has served on the Board of Directors since 2008.

Denise Deforest
Director of NursingDenise joined the Sandy River Center team as the Director of Nursing in March of 2022. Her clinical career started in Neuroscience which inspired a transition to Medical Rehabilitation. She has served in several leadership roles in the Acute Rehabilitation setting, including Director of Health Information Management, Quality Director, and Chief Nursing Officer. Denise is a second-career nurse who obtained her Associate’s Degree from a Community College, followed by her BS in Nursing at Drexel University. She obtained her Master’s degree in Nursing from Western Governors’ University. She presented original research at the Association for Rehabilitation Nurses (ARN) Annual Conference in 2013 and has volunteered as an ARN abstract reviewer in other years. She maintains her specialty certification in Nursing Rehabilitation.

Craig Hinkley
Director of Admissions & MarketingCraig joined the Sandy River team in 2020 performing multiple duties before transitioning into the Marketing and Admissions Director. Craig has a medical background as a licensed PTA graduating in 2019 from KVCC. He also brings along experience in marketing and sales with former businesses. Craig is Sandy Rivers lead team member for the Customer Excellence committee and Partner Program. In his spare time Craig enjoys fishing, hunting, bowling and spending time with his family. He is dedicated to making a smooth transition into Sandy River for patients and their families.

Deb Burchfield
Nurse PratitionerDeb Burchfield received her master’s in family nurse practitioner studies at St. Joseph’s College in Standish, Maine. She completed her doctorate in nursing from Chamberlain University. Deb is palliative care certified and has a passion for caring for the geriatric population with a focus on quality of life goals. Deb lives with her family in Farmington, Maine and has been serving the geriatric population for over 30 years.

Dr. Osella
Medical DirectorDr. Osella attended Fordham University as an undergraduate, earning bachelors in biology and sociology. From there, she studied at the Frank. H Netter, MD School of Medicine at Quinnipiac University to obtain her medical degree. She completed a residency in Internal Medicine at Maine Medical Center in Portland, ME before moving to Boston, MA as a geriatric fellow at Boston Medical Center. She is excited to be back in Maine and providing care to the rural geriatric population in Farmington. Other academic interests include medical education and effectiveness among interdisciplinary teams.
